It will likely be related to the ItemID.
If you look at the un-sef'd version of a URL you'll notice an ItemID, when you publish a module to a menu it links the module appearing with that ItemID.
I suspect that in some places you're linking to the page you expect to have the module show, without the ItemID being present in the URL, so it won't show.
